Hi Mark,
In this case, I would suggest using the Written Response question type, which will allow the students to use the HTML Editor to respond to the question. When responding to the question, the students can choose to insert an image to respond.
Note that another option would be to create an assignment folder and have students upload an image as their submission. this would enable you to use the annotation capabilities to annotate directly on the student submission.

Hi Mark,
To add to my response, the Written Response question type will allow students to insert a URL that points to their image (when they click the Insert Image button). They can also choose the Insert Link button to submit an image to the question via the OneDrive or Google Drive integrations.

What about uploading a pdf or Excel file?? I would like to do both for timed assessments (quizzes).

We tried to set up a quiz question this way, and found that our students only had access to add a Link to a file that was already hosted online at an image sharing site.
Not having some type of upload feature has made asking students to submit any type of diagram as a quiz question not possible.
We also really hoped that students would be able to use the Insert Stuff feature to upload .docx or .xlsx files but again the only useful feature is to insert a link to a file that's already been uploaded to the internet elsewhere.

Thanks for reaching out. The ability to upload files to quiz questions, including images and documents, is currently supported through the OneDrive/Google Drive integrations as well as adding a URL link to a file.
The ability to simply upload a file from the student's computer to a quiz question is something that's been requested within the Brightspace Product Ideas Exchange. Mat, I'm sure you are no longer following this conversation, but I had the same issue just now that you did. I was able to find the PIE item that Phil was referencing by just searching for the number (D2443) from his link. This is, I think, the correct link and it says that the item has been released, so you should have access to it.
